Before your pup or feline checks in for their staycation, we need a few quick forms filled out:

 

STEP 1:                                           -So we know who’s in charge of the treats!
 

STEP 2:                                           -The camp rules for happy campers.

 

STEP 3:                                           -All the details to make their stay paw-fect.
 

STEP 4:                                            Dogs Only. If your pup wants to join the playgroup fun during their stay!

                                                                 (We can do the temperament test when you arrive — easy peasy.)

​STEP 5: Email current vaccine records to info@altavistapets.com. (Distemper/Parvo, Rabies, Bordatella)
